OrsoBio Presents Positive Phase 2a Data for TLC-2716 in Severe Hypertriglyceridemia and MASLD

OrsoBio, a clinical-stage biotech, reported that its experimental drug TLC-2716 cut triglyceride levels by up to 57% compared to placebo in just four weeks, according to BusinessWire. The company shared the Phase 2a results at the Endocrine Society's Annual Meeting (ENDO 2026) in Chicago on June 14, 2026.
The drug also slashed remnant cholesterol — a fat particle tied to heart disease — by more than 50%. It reduced liver fat by an average of 8.4 percentage points. OrsoBio CEO Dr. Mani Subramanian called the results "a paradigm shift" in treating severe metabolic disorders.
TLC-2716 belongs to a class called LXR inverse agonists. LXR stands for Liver X Receptor — a protein in the liver that controls how much fat the body makes. Earlier drugs tried to activate LXR to move cholesterol out of cells. That approach backfired badly. It caused the liver to produce massive amounts of fat, making patients worse.
OrsoBio took the opposite approach. TLC-2716 blocks LXR activity instead of boosting it. That shuts down a process called "de novo lipogenesis" — the liver's way of making fat from scratch. The result is less fat in the blood and less fat stored in the liver, according to BusinessWire.
The Phase 2a study enrolled 64 patients. Their average starting triglyceride level was 612 mg/dL — well above the 500 mg/dL threshold that marks severe risk for pancreatitis, a painful and potentially fatal inflammation of the pancreas. The average body mass index was 34.2, placing most patients in the obese range, per data presented at ENDO 2026 and reported by BusinessWire.
At the highest dose, triglycerides fell 57% more than in the placebo group. No patients showed dangerous liver enzyme spikes — a key safety concern for LXR-targeted drugs. LDL cholesterol, often called "bad cholesterol," also stayed stable. Principal investigator Dr. Seung-Hee Choi said she had "rarely seen a 57% reduction in triglycerides over just a four-week period" in twenty years of practice.
About 4 million Americans live with severely high triglycerides. Standard treatments like statins often fail this group. High triglycerides also frequently appear alongside MASLD — a condition where excess fat builds up in the liver. MASLD now affects roughly 30% of the U.S. population. No single approved oral drug currently targets both problems at once, according to Yahoo Finance.
TLC-2716 is taken by mouth, which sets it apart from the injectable drugs that currently dominate the severe lipid treatment market. The FDA gave TLC-2716 Fast Track Designation in August 2024, recognizing the urgent need for better options for patients at risk of life-threatening pancreatitis.
OrsoBio plans to launch a larger Phase 2b trial, focused on reversing the more severe liver condition known as MASH (Metabolic Dysfunction-Associated Steatohepatitis), by the fourth quarter of 2026. That study will run 24 weeks — six times longer than the current data window — and aim to show lasting liver improvement, per BusinessWire.
Some experts remain cautious. Critics note that four weeks is too short to rule out effects on the brain or hormone systems, since LXR also plays a role in those areas. The FDA is expected to require neuro-cognitive and bone density testing in Phase 3 to confirm that TLC-2716 truly stays in the liver. Those answers will likely arrive with Phase 2b data in 2027.
