Danish Study Finds Specific Workplace Activities May Raise Miscarriage Risk in Early Pregnancy

The researchers developed an “improved pregnancy-specific job exposure model” that combined activity-tracker readings with expert evaluations of time spent standing, walking, and forward bending (at a 30-degree angle or more).

Walking and standing also showed links to miscarriage risk, though smaller ones. Each additional hour of walking per day was tied to an 18% higher risk, while standing added just 3%. Researchers stressed that normal everyday movement is unlikely to be dangerous — the findings target sustained occupational exposure, not a single bending episode.
The Danish team built what they called a "pregnancy-specific job exposure model." It combined activity-tracker data from 1,438 workers with expert evaluations to estimate how much time women in different jobs spent standing, walking, or bending forward at 30 degrees or more. This made the exposure estimates far more precise than older methods, according to JAMA Network Open.
Lead author Lærke Mygind, PhD, of Aarhus University, clarified what the 36% figure actually means. It reflects an extra full hour of bending across an eight-hour working day — sustained throughout early pregnancy. It does not mean a single bend is harmful. The risk was even higher, with a hazard ratio of 1.48, among women who had already been absent from work in the weeks before a miscarriage.
Researchers offered two possible explanations for how bending could affect pregnancy. First, repeated forward bending may raise pressure inside the abdomen. That could reduce blood flow to the uterus at a critical time — the first trimester, when the placenta is still forming. Second, physical strain may interfere with hormonal signals needed to support early pregnancy.
The study is observational, meaning it cannot prove cause and effect. The authors warned of "residual confounding" — gaps in the data. Factors like smoking, caffeine intake, alcohol use, and conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders were not fully accounted for. Women in physically demanding jobs may also face other stressors that contribute to miscarriage risk, JAMA Network Open noted.
The study recorded a miscarriage rate of just 6.1%. That is far below the commonly cited rate of 10–20% in the UK, as referenced by the Royal College of Obstetricians and Gynaecologists. Researchers explained the gap: the Danish registers only captured miscarriages that led to hospital contact. Many early losses — particularly those before 8–10 weeks — are managed at home and never appear in official records, according to EurekAlert.
Other known miscarriage risk factors were noted alongside the findings. These include smoking, night shift work, older parental age, air pollution exposure, and certain chemical compounds. The study's authors did not suggest bending replaces these as the primary concern — rather, it adds to the list of occupational factors worth tracking.
The research team was explicit about their goal: they want occupational health guidelines updated to better protect women in the first trimester. Current Danish and EU guidelines focus heavily on heavy lifting and night shifts. Jobs in nursing, retail, and industrial cleaning — where bending and walking for hours is routine — have largely been overlooked, devonlive.com reported.

Mygind emphasized the findings are not a reason to fear normal movement. "Everyday movement in one's free time" is not the concern, she said.
